LMC Foundation Check Request Form

Use the LMC Foundation Check Request Form when you would like the LMC Foundation to print and mail a check from your foundation account. You must provide back-up documentation and your dean\department chair needs to sign the Check Request Form.

Check requests received before 5:00 p.m. on Fridays will be printed and mailed the following week.

LMC Foundation Deposits

Use LMC Foundation Deposit Form when making a deposit to a Foundation Program Account. Fill out and print the form and send the associated checks to the LMC Foundation Office for deposit as soon as possible.  We want to thank donors promptly for their donations.

LMC Foundation Cash Deposits

The LMC Foundation accepts no cash. Bring cash to the LMC Cashier's office along with a completed copy of the LMC Foundation Cash Deposit form.

LMC HOSTS (Helping Our Students to Succeed) Referral

LMC employees can refer students to receive a one-time emergency scholarship up to $500 by completing the LMC Foundation HOSTS Referral Form.  The scholarship is available to any continuing student who is enrolled in at least six units and is in good academic standing.  Funds can be applied towards the cost of books, supplies, and/or emergency life events such as childcare, food, rent or transportation.

Payroll Deduction Form

The LMC Foundation Payroll Deduction Form can be found on InSite in the HR-Payroll tile and is for faculty and staff – full time and part time – who would like to set up automatic, payroll, tax deductible donations to support the LMC Foundation. You can designate where your funds will be used such as an  LMC Foundation Scholarship, Program Account or Wherever the Need is Greatest  and the dollar amount ($1.00 minimum) to be deducted from each paycheck. You can set up contributions of any amount to a single or multiple scholarship or program accounts.
